While this report is anonymous, you may choose to include contact information for follow-up and further investigation. These reports go to the Executive Vice Dean’s Office and are then fully de-identified and reviewed by the Learning Environment and Professionalism Subcommittee. You are encouraged to contact the course/clerkship director or any of the assistant or associate deans (Education or Student Affairs) if you are unsure if an instance constitutes mistreatment and would like to discuss before submitting.
